2. Fill the prescribed application form carefully in Hindi/English with all mandatory details as per instructions.
3. Attach self-attested copies of required documents:
   – Permanent resident certificate of Madhya Pradesh (for MP candidates).
   – Disability certificate (if applicable, for MP candidates).
   – Caste certificate (for reserved categories of MP).
   – EWS certificate (if applicable, for MP candidates).
   – Degree/Diploma certificate or provisional/final semester mark sheet showing CGPA/total marks.
   – 10th and 12th mark sheets.
   – Any other documents as specified in notification.
4. Paste recent passport size photograph in the space provided on the application form.
5. Mention clearly on the envelope the branch and category for which apprenticeship training is sought.
6. Send the complete application by post in prescribed format so as to reach:

7. Applications received after the last date or with incomplete/incorrect information or without required self-attested documents will be rejected and no correspondence will be entertained.
